Medical Exam Walk Through
Tips
1. Bring all the requirements needed as stated in the clinic's website and a black ball pen to sign forms.
2. Make sure to eat before going to the clinic as food is not allowed in the clinic. We highly recommend that you bring a bottle of water. Snacks that will fit in your purse may be taken into the clinic but be a good neighbor and dispose of your wrappers and trash in the provided trash reciprocals.
3. Bring a jacket or sweater, something that will keep you warm. It's pretty cold inside as they run the a/c at a low temperature.
4. Bring a book or something that'll keep you busy while waiting.
5. Be prepared to make new friends!
6. Please check your PDS form and make sure all laboratory and examinations has a Nurse or Doctor's initials or signature. Having their initial or signature means you have completed that exam otherwise you will need to go back and determine the issues to secure a signature.
Here's is the medical examination process: (Waiting time may not be the most accurate but it is good enough for government processing)
Your First Day
1. Arrive at the clinic around 4 am. The line begins forming very early and don't be surprised if there is already a line of applicants before you. Upon arrival at SLEC, applicants WILL BE ACCOMMODATED ON A FIRST-COME, FIRST-SERVED BASIS. NO APPOINTMENT IS NECESSARY that's the reason why people begin to line up at 4 am. Two lines will form, one for US applicants and one for Canada, Australia, New Zealand applicants. Be sure you are in the right line.
Waiting time to #2 below: approximately 1 hr. 30 mins.
2. Around 5:30 am, the guards started accepting applicants. Make sure to have your papers ready. The will be looking for the required documents but more specifically, two copies of St. Luke’s online registration form, two copies of the first page of Appointment Confirmation page, a copy of your NOA2 from NVC, a photocopy of your passport biographic page, and your passport. The guard will check the papers and hand you a Patient Data Sheet (PDS) form to fill-out and you'll receive a "lucky" red-colored number. You then will move to the inside to the ground floor waiting area, where you will fill out the PDS form (aren't you glad you brought that pen) and wait.
Waiting time to #3 below: approx. 30 mins.
3. Around 6:00 am, U.S. reception counters started calling out numbers. When it your turn, you will hand all my papers to the cashier. She will ask for your name, birthday, educational attainment, and civil status. After, she will now take a picture of you and a finger print scan. Then she hands back to you your PDS form with your patient number and she will instruct you to go to 5th floor for payment.
4. Few applicants are waiting in line so it should not take long for you to make your payment. After paying, you will be asked to go to the X-ray on the 3rd floor. The lady will take your PDS form and when called you proceed to the X-Ray room. The waiting time varies depending where you are in the line from outside.
5. After the X-Ray, you will be told to go 5th floor for Vital Signs, Eye Test, Height, Weight and Physical Exam. This has the most crowded area and the most waiting time. You can wait upwards of 30 mins. or more to have your vital signs, eye test, height and weight taken. Leave your PDS form with the nurse and wait to be called.
Waiting time to #6 below: Approx. 1 hour.
6. Once vitals are taken you'll be called up to produce your PDS form and given a number for the physical examination. NOTE!! Don't worry ladies you'll be given a female doctor. :-) You'll wait 10 or so minutes then you'll go inside to the doctor's office.
The doctor will ask you for some medical history and current medical conditions. Be sure to honestly tell them any medical conditions that exist currently or in the pass. You will disrobe and have a paper smock in which to cover up with, then the Doctor will proceed with the exam. Typically, it takes about 15 minutes to complete. The doctor then will instruct you to go to 2nd floor for the Immunization Interview.
Waiting time to #7 below: Approx. 20 mins.
7. At the second floor, you will be given a form to fill-out and wait for your name to be called. The doctor will ask for some personal details (to ensure I'm the correct patient she's looking at on the computer), my vaccination history. She will enter information into your record on their computer and then will inform you that if your lab tests are good she'll tell you about the vaccinations you will receive. Then you will be told to go to 4th floor. (all test and vaccinations are included in your medical fee so there is no additional fees to pay for the standard visa examination procedure)
Waiting time to #8 below: Approx. 20 mins.
8. At the 4th floor, you'll have a urine test and blood extracted, then waiting for about 15 minutes. The nurse will then instruct you to return tomorrow at 7:30 am for your results and completion of your vaccinations. They will remind you to bring your passport and 2 pcs. 2x2 passport photos.
Your day will vary, but if you are toward the front of the line you should be out between 10 am and 12 noon. If it is very busy it can go till 3 - 4 pm.
Second day
1. Plan to arrive at SLEC around 7:20 am. Proceed straight to the guard and show your receipt from the previous day. You do not need to line up again like the previous day, just proceed to the guard. The guard will check your PDS number on the computer and stamped coding on the PDS form and then instruct you to go to 2nd floor for Immunization.
Waiting time to #2 below: Approx. 1 hour.
2. Hand the nurse your payment receipt and wait to get your results. You will be called to the Immunization room to get your shots, and you will be told not to get pregnant in the next 30 days. The nurse will direct you to the ground floor for releasing. Do not leave the building till you have been told to do so.
Waiting time to #3 below: Approx. 35 mins.
3. The nurse will have you sign some papers. Then you sit down and wait for 5 to 10 mins at which time you will be called again at one of the releasing counters and asked for your passport, 2 pcs of the 2x2 passport photos and then asked for some personal details to confirm your identity from the papers she's holding.
Then she will have you place your finger on the fingerprint scanner and your patient profile will appear on her computer. She then double checks everything and asked you to also double check your information on the screen and papers she will show you.
Once everything is done and found correct, she then will ask for your interview date and you will be given your medical results then to take with you but on occasion they will forward the Medical results to the embassy. That's been a new policy but most often you will carry your records to your interview so DON'T LEAVE THEM ON THE BUS, IN A TAXI OR ON THE AIRPLANE!!!!!
She will give you copy of the vaccination form DS-3025, and you are to create copies of it, and send a copy via email to your fiancée as you need it once you’re married and filing for your AOS.
She should hand you your passport back (please don't leave without your passport!!!!) and a CD with your X-ray results. (You will never need that CD every again, so file it away or use it for a coaster for your drink) After the explanations are given you will be told you are finished with your medical exam and you can now leave. DO YOU HAVE YOUR PASSPORT???
